Social Worker or Public Health Nurse - Aging & Assessment

Job Summary

To provide MnCHOICES assessment services to the elderly and disabled population to determine eligibility for State Waiver programs, PCA, and Rule 185 Case Management.  To develop and coordinate a person-centered support plan with individuals, their families, and support network.  To provide direct support to individuals that address the bio-psychosocial wellbeing of the person that remains in compliance with county state and federal rules and statutes.  To provide mandated services as required by statute.

 


Essential Duties and Responsibilities

These examples of typical duties do not include all possible tasks for this classification and do not limit the assignment of related tasks.  Regular attendance according to the department's approved work schedule is required for all Sherburne County positions. 

  • Carry out the responsibilities of the lead agency for conducting MnCHOICES assessments to determine eligibility for State funded programs.
  • Complete person-centered Community Support Plans that reflect the needs of the individual that were identified through the assessment process.
  • Participate in regularly scheduled interdisciplinary team meetings to review service eligibility.
  • Complete assessments, receive referrals for services and determine eligibility
  • Complete referrals as appropriate
  • Ensure data is kept confidential per HIPAA and data privacy standards.
  • Provide outreach services to target populations.

 


Minimum Qualifications

 

If a Public Health Nurse:  Bachelor's degree in Nursing or higher degree with a major in nursing (no related field allowed or accepted) and completed course work which included theory and clinical practice in public health nursing.

  • Current licensure by the State of Minnesota as a Registered Nurse, certification from the Minnesota Board of Nursing in Public Health Nursing.
  • Must currently be a MnCHOICES certified assessor or have 1 year of experience in Home and Community Based Services to become a certified assessor.
  • Current Minnesota driver's license.
  • Preferred: Current Certified Assessor.

If a Social Worker: Must have a Bachelor's degree or higher in Social Work or a closely related field. Plus 1 year of social work experience, or equivalent education plus work experience.

  • Must currently be a MnCHOICES certified assessor or have 1 year of experience in Home and Community Based Services to become a certified assessor.
  • Current Minnesota driver's license.
  • Preferred licensure: Current Certified Assessor, Licensed Social Worker (LSW), or greater.

Hiring Salary Range
$29.88 - $35.86/hour

Hours of Work
This position has a usual work schedule of 8:00 a.m. to 4:30 p.m., Monday through Friday; may include occasional evening and weekend hours.
