Community Health Coordinators protect and promote the health of individuals, organizations and communities in Sherburne County by advocating for and supporting healthy lifestyles, policies and environments. This position will be primarily focusing on Substance Use/Alcohol, Tobacco and other drugs.

This position is located at the Government Center in Elk River, MN. Our regular business hours are Monday-Friday 8:00am-4:30pm, though the department provides staff with flexibility in effort to strive for a desired work/life balance.  Essential Duties and Responsibilities
These examples of typical duties do not include all possible tasks for this classification and do not limit the assignment of related tasks.  Regular attendance according to the department's approved work schedule is required for all Sherburne County positions.

  • Determine current health status concerns, imminent risks and readiness of the population as it relates to health promotion issues.
  • Use data to determine health strategies to decrease risk, increase knowledge base of preventative and imminent health issues to aid in the development of reduced risk lifestyles. 
  • Assist in writing grant applications. 
  • Implement population level health promotions to address health issues in targeted audiences. 
  • Deliver education and training and provide resources and technical assistance in various topics such as; nutrition, physical activity, alcohol, tobacco and other drug prevention and control, traffic safety, and prevention of youth risk behaviors. 
  • Advocate Policy, System and Environmental Change work. 
  • Coordinate and monitor health promotion activities. 
  • Serve as a consultant and resource for internal and external groups and organizations.
  • Engage community partners.
  • Monitor health promotion programs and strategies. 
  • Provide progress reports and outcomes, including grant reporting.
  • Responds 24/7 as needed during a public health emergency in designated roles.
Minimum Qualifications
Education: Bachelor's Degree in Community Health, Nursing, or other directly related field.

Experience: Minimum of one year experience in Health Education or Health Promotion.

Must have a valid MN driver's license.
 
Plus factors that will be considered include CPR certification, Child Passenger Safety Technician Certification, CHES certification, work experience in community health, adolescent health education, employee wellness programs, delivering presentations in a public forum, compiling and preparing statistics and narratives, preparing grant applications and conducting program evaluations.

2024 Hiring Salary Range
$32.01 - $38.41

Hours of Work
This position has a usual work schedule of 8:00 a.m. to 4:30 p.m., Monday through Friday; may include occasional evening and weekend hours. 

Work Location
Main office is at the Government Center in Elk River, MN. After 6 months position has the ability to become hybrid. 

Interviews
Selected candidates will be notified via email November 20th-22nd for 15 minute phone interviews on December 5th.

Typical Working Environment:
100% inside work environment.  
 
This position is subject to moderate office noise levels. There is some mental stress involved resulting in inconvenience and frustration. 
 
Position requires contact to staff within the Health & Human Services department and other departments, outside agencies, and the general public (supplying or seeking information) on specialized matters. Contacts may include clients, co-workers, lead workers, management, hospitals and clinics, and other agencies.  

In all settings, members of the public who exhibit challenging, atypical or hostile behaviors and/or communication.
                                                                    
Typical Physical Requirements for this Position:
  • Hand/eye coordination. 
  • Ability to lift 20-30 pounds.
  • Engage in the following movements: Climbing, balancing, stooping, kneeling, crouching, reaching, standing, walking, lifting, fingering, grasping, feeling and writing and repetitive motions. 
  • Exert up to 30 pounds of force occasionally, and/or a negligible amount of force constantly to move objects. Moving and installing child restraint systems in vehicles.  Ability to lift 40# to carry supplies, display boards and health education materials.   
  • Hearing ability sufficient enough to communicate with others effectively in person and over the phone 
  • Visual ability must be sufficient enough to read typewritten documents, computer screen and drive a car.
