Join the team of Social Workers in the Child Protection unit of the Sherburne County Human Services! 
Our team of knowledgeable and experienced Parent Support Outreach Program Social Workers conduct comprehensive assessments of families’ needs and strengths and, together, make decisions about what services or community resources are the best choices for success.

This position is located at the Government Center in Elk River, MN, however will also require work in the field. Our regular business hours are Monday-Friday 8:00am-4:30pm, though the department provides staff with flexibility in effort to strive for a desired work/life balance. Essential Duties and Responsibilities
These examples of typical duties do not include all possible tasks for this classification and do not limit the assignment of related tasks. Regular attendance according to the department's approved work schedule is required for all Sherburne positions.
 

  • Complete assessment for case management and other service needs when reports of prenatal exposure have been received and Parent Support Outreach Program (PSOP) referrals made involving risk factors involving parental substance abuse.
  • Make the necessary referrals to Public Health and other community resources as needs are identified.
  • Develop Safety and Self Care Plans. 
  • Complete pre-petition screening consultations when a pregnant mother is refusing recommended services, fails recommended treatment, or cannot be found and court intervention may be needed.  
  • Engage with prospective father’s when identified. 
  • Follow timelines and process as outlined in the Minnesota’s Best Practice Guide for Responding to Prenatal Exposure to Substance Use.
