Social Worker - CAC/CADI/BI and Developmental Disabilities

Job Summary
To provide professional case management to individuals who are eligible for CAC/CADI/BI Waiver services, Developmental Disability (DD) Waiver Services, and/or Rule 185 (Development Disability) case management. Mandated services are provided as required by Federal and State Statute and Rules. To develop and coordinate person centered plans with individuals with disabilities, their families and support network. Mandated services are provided as required by Federal and State Statute and Rules. To complete assessments to identified populations. To provide direct services of counseling, facilitation of interdisciplinary team meetings, coordination and consultation that address the bio-psychosocial well-being of the individual that remain in compliance with County, State and Federal Rules and Statutes.
This position is located at the Government Center in Elk River, MN. Our regular business hours are Monday-Friday 8:00am-4:30pm, though the department provides staff with flexibility in effort to strive for a desired work/life balance. Essential Duties and Responsibilities
These examples of typical duties do not include all possible tasks for this classification and do not limit the assignment of related tasks. Regular attendance according to the department's approved work schedule is required for all Sherburne positions.

  • Provide CAC/CADI/BI or DD Case Management Services
  • Complete assessments, receive referrals for services, and determine service eligibility and need.
  • Actively participate on various committees and provide outreach to target population.
Minimum Qualifications
Education: BA/BS in Social Work or closely related field.

Must have a valid MN driver's license and reliable vehicle.
Experience:  
 Social Worker I: Minimum of completed Social Work internship or closely related field
 Social Worker II: Minimum of 1-year of Social Work experience or equivalent education plus work experience
For DD positions: Applicants must meet qualifications for Qualified Developmental Disability Professional (QDDP).
 QDDP: Has at least one year of experience working directly with persons with intellectual disability or other developmental disabilities; and is one of the following:
 (i) A Doctor of Medicine or osteopathy.
 (ii) A registered nurse.
 (iii) An individual who holds at least a bachelor's degree in a professional category
Preferred Experience Beyond Minimum Qualifications: Candidates experienced with Vulnerable Adults, MMIS, and SSIS are preferred; as well as candidates licensed at LSW level or greater.

2024 Hiring Salary Range: 
Grade 12: 
$29.92 - $35.91/hour
Grade 13: $32.01 - $38.41/hour  
Vacancies: 3  (2 in Developmental Disability and 1 in CAC/CADI/BI)
Union: Teamsters

Hours of Work:
This position has a usual work schedule of 8:00 a.m. to 4:30 p.m., Monday through Friday; may include occasional evening and weekend hours.

Position Availability: Positions would not be eligible to start prior to 1/6/2025. Also, is dependent upon final Board approval.

Interviews: Selected candidates will be notified via email the week of 12/4 or 12/5, to self-schedule 10-15 minute phone interviews to be held on 12/9.  



Physical Requirements and Work Environment
Must be able to endure extended travel, up to 8 hours or more per day. Must be able to provide own means of transportation when county vehicles are not available.  May be required to travel to remote areas and in all weather conditions.
Must be able to exert very moderate physical effort in light work, typically involving some combination of kneeling, crouching, and crawling, and which may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (15-25 pounds).
 
Typical Working Environment:
80% Inside Work (office and field work), 20% Travel. (These percentages are an estimate and may vary based on job duties.)

Work is performed both in the office and in the field (some examples of settings include client’s: homes, apartments, hospitals, group homes, schools, shelters, correctional facilities, and treatment facilities).  Headaches, eyestrain, and other related occupational hazards reflect the most common potential for injury in the office.  Field visits to clients’ homes may expose employee to unsanitary conditions, communicable illnesses, infrequent exposure to health hazards, unpredictable and/or unsafe environments.
Likelihood of verbal abuse and threats from hostile, frustrated clients and may include unpredictable situations.
